Recently at the iconic TCL Chinese Theatre in Hollywood, I watched Dr. Valter Longo’s new documentary — Fasting and the Longevity Revolution, narrated by Edward Norton and directed by Academy Award-nominated Barry Alexander Brown — premiere to a room full of researchers, filmmakers, and health advocates. And I felt something I haven’t felt in a long time: hope that the mainstream is finally catching up.


Every point. Every mechanism. Every truth about what fasting does to the human body. Here’s what the film — and the science behind it — needs you to know.



Your Body Was Built to Fast

For most of human history, we ate when food was available — not on a three-meal-a-day schedule designed by the food industry. Between meals, the body shifted into repair mode. That mode has a name: fasting. And the cellular process it triggers has a name too: autophagy.

Autophagy is the body’s built-in cellular housekeeping system. During autophagy, damaged proteins and dysfunctional cells are broken down and recycled into raw material for new, healthy tissue. It’s nature’s cure — and it won the 2016 Nobel Prize in Physiology or Medicine.


When we eat constantly, we suppress this process. Insulin stays elevated. The cellular cleanup crew never gets to do its job. The result is accumulated damage that shows up as weight gain, brain fog, fatigue, inflammation — and eventually, chronic disease.


The Science of Fasting and Longevity: You Can Reverse Biological Age

Dr. Valter Longo, Director of the Longevity Institute at USC and one of Time Magazine’s 50 most influential people in healthcare, has dedicated his career to one question: how do we help people live longer — and live better?

Published in Nature Communications in 2024, Longo’s clinical trial found that three consecutive FMD cycles:

→  Reduced biological age by an average of 2.5 years

→  Improved metabolic and immune function

→  Reduced abdominal and liver fat

→  Lowered risk factors for cancer, diabetes, and heart disease


Fasting and Cancer: What the Clinical Trials Are Showing

One of the most groundbreaking — and underreported — areas of Longo’s research is what fasting does in the context of cancer treatment. His newest book, Fasting Cancer, published in February 2025, lays out decades of research on this. The core discovery is remarkable: fasting appears to make cancer cells more vulnerable to treatment while simultaneously protecting healthy cells from chemotherapy’s toxic side effects.

This is the differential stress resistance principle: healthy cells, when fasting, shift into a protected, repair-focused state. Cancer cells — which are metabolically rigid and glucose-dependent — cannot make that shift. They become more exposed, not less.


In the landmark DIRECT trial — a multicentre randomized Phase 2 clinical trial published in Nature Communications — 131 patients with early-stage HER2-negative breast cancer were randomized to follow the FMD for three days before and during neoadjuvant chemotherapy, or to eat normally. The FMD group showed:

→  3x greater odds of radiological complete or partial tumor response

→  4x greater odds of 90–100% pathological tumor cell loss

→  Significantly less chemotherapy-induced DNA damage in immune cells (T-lymphocytes)

→  Elevated levels of immune-fighting T cells, B cells, and natural killer cells that actively infiltrate tumors

→  Reduced fatigue and better quality of life during treatment

→  Better overall survival in advanced triple-negative breast cancer patients when FMD was added to carboplatin-based chemotherapy (International Journal of Cancer, 2024)

As Longo puts it: fasting doesn’t just protect the body during cancer treatment — it actively turns the immune system against the tumor. This is not fringe science. These are peer-reviewed, randomized clinical trials published in Nature Communications and leading oncology journals. And at least five ongoing clinical trials at major cancer centers are investigating this further.

 

The Missing Link: Insulin Resistance

A 2025 meta-analysis in Frontiers in Nutrition examined 10 randomized controlled trials: all fasting protocols significantly reduced fasting blood sugar, insulin levels, insulin resistance scores, and HbA1c compared to controls. And a landmark 2024 Chinese RCT found a 5:2 intermittent fasting protocol reduced HbA1c by 1.9% — outperforming both metformin (1.6%) and empagliflozin (1.5%).
